ACNE JUVENILE
On fatty skin, called seborrheic juvenile acne, or polymorphic acne occurs in both sexes at puberty.
The starting point is the comedo. Black comedones, a mixture of keratin and sebum, obstruct the orifice of the pilosebaceous follicles; Keratin or dust colors them in black. They can easily be extracted using appropriate equipment. White comedones, true microcysts obstructing follicular channels, resist any attempt to extirpate by pressure. It is principally from them that suppurative formations develop: pustules with the pilosebaceous orifice and in-depth small abscesses or larger inflammatory tuberosities, or indeed boils. The association of these various elements produces a polymorphic eruption, and according to the predominance of one of them, are described comedogenic, papular, tuberous, nodular, phlegmonous acnes.
Acne sits in selective areas of seborrhea. The face, the forehead, the nasogeneal region, the chin are mostly affected. Elsewhere, the interscapulo-vertebral and pre thoracic zones, sometimes deltoid, are affected.
Beginning at puberty, acne evolves by flare-ups, at the origins of which are evoked intestinal disorders, excess absorption of sweets or fat and, more sharply, a premenstrual upsurge.
Although it is common to see acne lessen at around twenty-five years of age, this development is not constant and recovery is possible.
Moreover, if it has not been treated, acne leaves indelible scars, sometimes very important, and then called "vermoulantes". These scars are favored by the manipulations that some make to their "buttons" and especially by the digital expression.
Generally considered benign, acne, highly unsightly, can provoke psychic disorders, especially in the excoriated acne of young girls, where on occasion of lesions sometimes minimal develops a real tendency to self-harm.
ACNE ROSACEA
Mainly in women around menopause, but sometimes in both sexes and at any age, acne rosacea can begin on dry skin. It is preceded by a stage of facial erythrosis occurring at first intermittently, during abrupt changes in temperature, absorption of alcoholic beverages, and a meal too abundant in premenstrual period or as a result of the various disturbances Psychological order. Congestive flushing is becoming more frequent and erythrosis becomes permanent. At the same time, fine telangiectasia appears, at first limited to the cheekbones and then more and more extensive. Conjunctivitis frequently associates with these cutaneous alterations. On this background are developed tiny papules and papulopustules which can occur by sudden surges, disfiguring the sick. The evolution is indefinite in the absence of treatment.
In other cases, acne rosacea develops on the oily skin, at any age, in subjects generally presenting juvenile acne, often male sometimes deforming the aspect of the nose (rhinophyma). Numerous etiological factors have been mentioned: digestive and circulatory disorders, in particular, neurovegetative imbalance, various allergies, in particular to Candida albicans, the role of the emotions.
A particular variety is symptomatic of an ariboflavinosis. It is accompanied by formations of crusts more or less thick and an ocular affection detected by the ophthalmological examination.
